Private Dental Insurance

We honor all forms off PPO dental insurance and as a courtesy to you, we will submit a claim to your dental insurance company following treatment.* Please provide us with your dental insurance card and any additional information we may need during your first visit. We recommend that you call your insurance company to verify your dental coverage. Remember, due to the vast differences in dental insurances, it is your responsibility to know your policy benefits and limitations.

^*Full payment of services rendered is due at the time of treatment for those individuals with out-of-network insurances. Insurance reimbursements may be sent directly to you from your insurance company or to our office. If we receive an insurance reimbursement check for you, we will credit your account or mail a reimbursement to you, upon your request.
